NEW PATIENT FORMS
Your New Patient Experience
Once you’ve decided to start your journey with us, the first step is to schedule a new patient consultation with Dr. Brian McGuckin, D.C., DABCI. During this consultation, we’ll have a chance to get to know you better, understand your health goals, and discuss your concerns.
We’ll assess if our services are the right fit for your needs. We’ll also discuss the costs associated with your care and determine what laboratory testing may be required.
The second appointment is the physical examination and to draw any necessary blood samples. Once we have the results, we’ll sit down with you for a report of findings, where we’ll discuss your current health status and work together to create a personalized care plan.
Convenient Testing Options
For your convenience, we offer on-site blood drawing services. Alternatively, we can arrange for a phlebotomist to visit your location or send you to a local Quest Labs for bloodwork. The choice is yours, and we’re here to make it as convenient as possible.
Pricing and Payment Options
We ask that you arrive ten minutes early to fill out paperwork or you can download our New Patient Forms to complete ahead of time.
Chiropractic Visits
You will have a consultation, examination, and adjustment that will up be to 30 minutes in total. The fee will be $100.00.
We do not require you to return at regular intervals. It’s completely up to you if you’d like to schedule care for future appointments.
Nutritional Visits
You will begin by having consultation up to 30 minutes to determine what steps should be taken to help you and the fee will be $100.00.

Payment Information
We ask that you pay at the time of service. We are an out-of-network provider. This means there may be a separate deductible or other restrictions on your reimbursement. We will happily file claims to your insurance company for your visits.
Saliva Testing and other lab work done at this office is considered nutritional testing by most, if not all insurance companies. There is typically no reimbursement for this type of testing. Anthem and Medicare specifically will not pay for laboratory testing done at this office.
